  • This form is to be used for all reports to USA Cheer in all categories of abuse and misconduct by USA Cheer members that are covered in the USA Cheer SafeSport Code.

    Do NOT use this form for:

    • reporting arrests (use this form)
    • USASF Members (use their reporting process)
    • other persons who are not members of USA Cheer (report them through the proper authorities, school, or local association)
    • reporting rules violations (follow the process of your local or state association)

    You may report ANONYMOUSLY. If you choose to report anonymously, please write ANONYMOUS where it says "Your full name".

    USA Cheer’s Policy on Anonymous Reports

    Anonymous reporting may make it difficult for USA Cheer to investigate or properly address misconduct or abuse, however, USA Cheer recognizes it can be difficult for an athlete, teammate, friend or family member to report an allegation of misconduct and strives to remove as many barriers to reporting as possible. Anonymous reports may be made by completing the Reporting Form without including their name (Enter Anonymous as both the FIRST and LAST name)


    To the extent permitted by law, and as appropriate, the U.S. All Star Federation and all U.S. All Star Federation Member Programs shall, on request from the complainant, keep the complainant’s name confidential, not make public the names of potential victims, the accused perpetrator or the people who made a report of child physical and sexual abuse to the authorities.
